Definition Interferons
are a group of proteins (20-30) called
cytokines produced by white
blood cells, fibroblasts, or T-cells as part
of an immune response
to a viral infection or other immune trigger.
The name of the
proteins comes from their ability to
interfere with the production
of new
virus particles or various antigens in the body.

Interferons ؟
مجموعة من
البروتينات يتم انتاجها كرد فعل للجسم عن
أي إصابة فيروسية أو أي خطر يهدد
مناعة الجسم
بحيث تمنع هذا الفيرس من الانتشار بتنبيه الخلايا
المجاورة
لتقاومه
كيف
يعمل Interferons؟؟
A cell infected by a
virus releases minute amounts of
interferons
which attach themselves
to neighboring cells,
prompting them to start producing their own
protective antiviral
enzymes.
The result is impairment
of the growth and
replication of the attacking virus.
عندما
تتعرض
خلايا الجسم لإصابة فيروسية تطلق Interferons كرد فعل بتركيز ضئيل
ليرتبط
بالخلايا
المجاورة وينبهها لوجود الفيرس بحيث تستعد لمواجهته وبالتالي منع
الفيرس
من الانتشار
Interferon has also been shown to have some
antitumor
properties. .
Your immune system
identifies and responds to the
presence of diseased or cancerous
cells by reading special marks on the
cell surface.
Once the "enemy" cells
have been identified, the
immune system sends out fighter cells to
destroy them.
Interferons are among a
number of substances
produced in response to the presence of enemy
cells. Not only do they
"interfere" with foreign invaders that may
cause infection, but they can
prevent the growth and spread of other
diseased cells as well,
including some types of cancer cells.
Interferons have been
shown to be effective against a
variety of tumors.
تنتج
Interferons كذلك في حالة وجود أورام وليس
الفيروسات فقط
فالـInterferons
تفرز من الخلايا في حال وجود أي خطر على الجسم ولا يشترط أن تكون فيرس
فيمنع
Interferons
انتشار بعض أنواع الخلايا السرطانية أوبعض أنواع الأورام

excretion take place mainly
in the liver and kidneys.
They rarely pass the
placenta and the blood-brain
barrier
Types
Interferon-alpha
Interferon-beta
Interferon-gamma
Alfa and Beta
interferons, which are
grouped together as type I interferon
are produced by white
blood cells and a type
of connective tissue cell called a fibroblast.
Gamma interferon (type
II interferon)
is manufactured T-cells
Production occurs when
the T-cells are activated such
as during an infection
Purpose
Interferons affect the
immune system in a number of
ways.

types of
cancer and tumor
AIDS
some
kinds of
hepatitis

الألفا لعلاج بعض أنواع السرطان
والأورام وبعض أنواع التهاب الكبد
الوبائي وكذلك لعلاج الإيدز
Beta interferons are used primarily in the
treatment of multiple sclerosis
enhance the activity of
lymphocyte
cells while simultaneously inhibiting other immune cells
from becoming
stimulated.
interferon
beta regulates the production of
interferon gamma.
تستخدم
البيتا
في علاج تصلب الأنسجة وتنشيط lymphocyte التي تقوم بدورها بتثبيط
عمل
خلايا مناعية أخرى فتمنع استثارتها
ويتحكم
النوع
بيتا في انتاج النوع جاما
Gamma interferons are
used in
*the treatment of *CGD

CGD*=Chronic Granulomatous Disease
is an
inherited
disorder in which white blood cells lose their ability to
destroy
certain bacteria and fungi
It is a
genetic
disease, meaning the defective gene is carried on
the X
chromosome

interferons used as Anti-tumor
It
is still unclear
They may make
surface-exposed
antigens of tumors even more capable of stimulating
the immune system,
which in turn would elicit a greater response
from the T-cells of the
immune system.
Tumor
growth may also be slowed
or retarded by interferon-mediated damage
to the blood cells that supply
the tumor with nourishment.
طريقة عمل الانترفيرون على
الخلايا
السرطان او الأورام ليس واضحة حتى الآن
ولكن يعتقد انها تؤدي لتحفيز جهاز المناعة وخلايا T
بشكل أكبر للقضاء على الورم
أو ربما
تبطئ نمو خلايا المرض
أو ربما
تمنع التغذية الواصلة لخلايا
الورم
How
is Interferon
administered?:
Interferons
are normally injected
They are not
taken by mouth
as the strong digestive
enzymes
of the stomach will
degrade them
For
use in multiple sclerosis,
interferon beta-1a is injected into the
muscle (intramuscular
injection), and beta-1b is injected just below
the skin (subcutaneous
injection).
Dosing
is based upon the disease
being treated and the interferon being
used.
The interferons may be given
intramuscularly,
intravenously, or subcutaneously on a daily or three
times a week
basis
New preparations of intron and
roferon
that
can be administered once
per week are now becoming available
تستخدم الانترفيرون عن طريق
الحقن
ولا تؤخذ عن طريق الفم لأنها قد
تهضم بواسطة
إنزيمات المعدة
فعلى
سبيل المثال في حالة علاج تصلب الأنسجة الذي
أشرنا له سابقاً في
الاستخدامات
والذي يعالج بواسطة النوع
بيتا
يتم حقن الانترفيرون بيتا اما في
عضلة أو تحت
الجلد او في الوريد
تحدد
الجرعة على حسب نوع المرض ونوع الانترفيرون
المستخدم
فقد تكون الجرعة يومية او يوم بعد يوم (بحيث تصبح
3مرات
أسبوعياً) أو ربما مرة فقط أسبوعياً

All interferon preparations
except
for two are stored in the refrigerator 2-8°C before and after
mixing.
Intron-A) may remain at room
temperature 30-35°C
for 7-14 days before mixing
interferon alfa-2b
may remain at
room temperature 15-30°C before mixing.
Both
of these latter interferons
should be used immediately or
refrigerated after mixing with the
diluent.
No
vial of interferon should be
frozen or shaken since this can damage
the interferon and make it
ineffective
جميع الانترفيرون تحفظ عند درجة حرارة 2-8 درجة
مئوية
ماعدا Intron-A يمكن حفظها عند درجة
حرارة 30-35
درجة مئوية لمدة 7-14 يوم
وتلك المشتخرجة من الحيوانات تحفظ عند درجة حرارة
الغرفة
لمدة 15-30 يوم
كلاهما قبل الخلط ولكن
بعد خلطها يجب حفظها في
الثلاجة
يراعى
عدم تجميد
الأنترفيرونات اوهزها بقوة لأن هذا ممكن ان يؤثر على فاعليتها
Precautions
Patients who
have had seizures or who are at
risk for a seizure should be closely
monitored following the
injection of interferon, as should those with
heart disorders such
as angina, congestive heart failure, or an
irregular heartbeat.
يجب
متابعة مرضى القلب عن كثب بعد حقن الانترفيرون
It
is not known if interferon
can be expressed in breast milk.
[size=16][size=16]Because of the potential for side effects in
the
infant, breast-feeding should be avoided by nursing mothers who
require therapy
with interferon
No
adequate studies have been done in pregnant women
with any of the
interferons; however, the use of interferons
during pregnancy is
best avoided because studies
in animals using doses more than 100 times the doses
used in humans
have shown an increased risk of spontaneous
abortions.

الأن
حول ضرر الانترفيرونات على الحمل ولكن يفضل تجنب استخدامه مع الحمل
لأن
الحيوانات المعملية اظهرت اجهاض عند استخدام الدواء معها بجرعات اكبر
مما
يستخدمها الأنسان ب 100مرة
كما ان المعلومات غير متوفرة حتى الأن عن
استخدامه
مع الرضاعة لذلك خوفا من الأضرار المحتملة يجب عدم استخدامه
DRUG INTERACTION
Alpha interferons can interact
with several
different drugs
Ex:
Some
of the depressants include
antihistamines, sedatives,
tranquilizers, sleeping medications,
prescription pain medicines,
seizure medications, muscle relaxants,
narcotics, and barbiturates.
Prior
to treatment, the doctor
should be notified if the patient is
taking any of these medications
because this could impact the dosage
prescribed
الفا انترفيرونات تتفاعل مع
كثير من الأدوية خاصة
الأدوية التى تعمل على الجهاز العصبى مثل المهدئات
والمنومات وادوية
الصرع
ومضادات الحساسية وبعض المسكنات
ومرخيات
العضلات و البارابتيورات لذلك يحب ان يكون الطبيب على درايه
اذا كان المريض
يستخدم احد هذة الأدوية
Interferon alfa-2a, interferon
alfa-2b
and interferon beta-1b may
increase blood
levels of
zidovudine (AZT, Retrovir). While this reaction may
improve zidovudine's
effectiveness
it
also may increase the risk of
blood and liver toxicity.
Therefore,
the dose of
zidovudine may need to be reduced by as much as 75%.
كما ان انترفيرون الفا
وبيتا
يزيد من تركيز ziduvudine فى الدم
مما يؤدى
الى تسمم الدم والكبد
لذلك جرعة zidovudine
يحب ان تقل بنسبة 75%
Interferon alfa-2a and
interferon
alfa-2b may increase the time it takes for theophylline to be
eliminated from the body
Therefore, the dose of
theophylline
may need to be reduced.
كما
ان الأنترفيرونات تزيد من الوقت اللازم
لخروج
theophylline من
الجسم
مما يزيد من تركيزه فى الدم لذلك يجب ان تقل جرعته

| SIDE EFFECTS OF INTERFERONS
The
degree and severity of the side effects
depend on the
amount and schedule of
the administration of
Interferon.
Flu-like
symptoms
fever
chills
headaches
muscle aches
Digestive
discomforts
nausea
lack
of appetite
vomiting
diarrhea
Mood
disturbances
depression
suicidal thoughts and
behavior

انتحارية
Blood disorders
Anemia
Low platelet count
Low
white blood count
Fatigue
and low energy levels
Hair
thinning (alopecia)
Injection
site discomfort

When
are flu-like
symptoms likely to happen?
They
are most
likely to start within the first two weeks of your
Interferons
injections.
They
may
occur shortly after your injection
How do you
deal with
it?
Adjust
the
time of your injections
taking them
just before bedtime may help you
sleep through the symptoms altogether.
Take
acetaminophen or another type of
analgesic for the fevers and aches.
Drink plenty of fluids
before and
after your injection.
Cut back on
caffeine and alcohol consumption.
تبدأ الأعراض
الجانبية
الشبيهة بأعراض الإنفلونزا خلال أسبوعين من بدأ العلاج
بالأنترفيرون
ينصح بأخذ
الانترفيرون قبل النوم حتى لا تؤثر تلك
الأعراض على نشاط خلال اليوم ويمكن
استعمال acetaminophen كمسكن للآلام
وخافض للحرارة
اشرب
الكثير من السؤال وابتعد عن المنبهات

How do you deal with nausea and vomiting?
Eat small
meals of easily digested foods
poached eggs,
fruits or vegetables, grilled
chicken.
Eat
dry foods
— toast or crackers — and avoid spicy, sweet, or greasy
foods.
Drink
plenty
of clear, cool beverages, such as water or apple juice.
Avoid sights
or smells that may nauseate you.
There are
anti-nausea medications that may be
appropriate for you.
لتجنب
الأعراض
الجانبية على الجهاز الهضمي تعود على أكل الوجبات الخفيفة
التي تجتوي أطعمة
سهلة الهضم
وتجنب الروائح
التي قد تجعلك تصاب بالغثيان

How can you deal with mood problems?
Talk about
them openly starting with your doctor and nurse.
Take
advantage of community social services
there are
usually plenty to choose from.
Talk to your
priest, minister, or rabbi.
Join a
patient support group for firsthand
insights and tips on coping with
your treatment.
Keep a
calendar and try to stay mentally
organized.
Carry
a
notebook and jot things down if you think you might forget them.

your injection is administered
You might
experience redness, dryness, itching
By rotating
your injection site to a different
spot each time

important to find ways to come to terms
with them
It
helps to
keep the following general guidelines in mind:
Try to
maintain an active lifestyle.
There will always be the temptation to sit around
doing
nothing — and that's okay, sometimes — but by keeping both your

and mind engaged, you keep the side effects from gaining the upper
hand.
Start an
exercise program , regular exercise will
help relieve your stress, boost
your energy, and improve your sense
of well-being. Your doctor will
help you design a program that's
realistic and effective.
Rely
on your
support team

الأعراض السابقة حافظ على نظام حياتك
سوف تشعر
أحياناً بعدم الرغبة في عمل أي شيء
اشغل عقلك
وجسمك بشيء ما لتمنع تلك الأعراض الجانبية
من أن تصبح المسيطر |
